When should I get medical attention for a pinched nerve?

I have a Sharp pain in my right shoulder blade area that I assume to be a pinched nerve. Pain 'radiates' around the spot and my right arm/hand is tingly/semi-numb. The pain started happening Thursday morning, it's now Sunday afternoon an getting worse. Pain killers havent done anything... Doctors appt? Thanks!
